This questionnaire will be used by PurRydeRoc as a memory aid mainly and as a means of determining if you will provide a suitable home for a PurRydeRoc kitten or cat. It is strictly for my personal use and the information that you provide will remain completely confidential. Please provide detailed answers particularly about yourselves, the living arrangements that you will be providing to a PurRydeRoc kitten or cat, and how he/she will be raised and treated as a member of your family. Please provide detailed information about past and present cats and other pets that have been or currently are members of your family. Our cats and kittens are not a commodity to us. They have been raised to become members of your family, even those that will become show cats or part of a breeding program. Completing this questionnaire will go a long way to having your request for a kitten or cat considered seriously thus adding your name to our list of future PurRydeRoc Maine Coon owners.

8.        Is the cat to be a pet for you or a gift for someone else?

9.        How much time would the cat spend alone each day?

10.      Where would the cat spend its day?  Where would it spend its night?

11.      Who would be the primary caretaker of the cat?

12.      Is there any one in your house that does not want a cat?

13.      Describe any additional requirements you have in a Maine Coon kitten.

14.      List all persons residing in your home, their age and occupation (if applicable).

15.      Do you own your own home or do you rent?  How long have you lived at your current residence? If less than two years, how long did you live at your prior residence?

16.      Do you live in a house, mobile home, apartment, or condo?

17.      If you rent, will your landlord allow pets?  Would your landlord provide a letter stating that he/she allows pets?

18.      If you belong to a home or condominium association, does your association allow pets?

19.      Does your home have screens on all doors and windows?  If not, how do you ventilate your home during the warm, summer months while still keeping the cat secure inside?

20.      Do you plan on moving in the next year or in the near future?

21.      Do you plan on starting a family in the future? If so how will a cat fit into these plans?

22.      List all the pets in your home along with a description of the pet, their personality, and their age.

23.      Have you ever owned a cat before?  (if so, how long and what happened to the cat)

24.      Do you have cats living with you now?  If so, how many and where did they come from?  What breed?  How old?  Are they altered?  Are they de-clawed?  Are they in good health?  What kind of cat food do you feed? Are they current with their vaccinations?  Are they indoors only?  Indoor/outdoor?  Outdoor only?  If kept indoors, how often do you clean litter boxes?

25.      Do you currently have a veterinarian?  Would your veterinarian provide PurRydeRoc with a reference?

26.      Have you contacted other Maine Coon breeders?  If so, who?

27.      How did you hear about us?  Cat show, newspaper ad, referred by someone (who?).

PurRydeRoc Maine Coons cannot guarantee a cat/kitten to everyone that fills out and responds to this questionnaire. We only have a few litters of kittens per year. The intent of this questionnaire is solely to determine if you would provide a loving home environment for a cat or kitten with adequate care and medical support should one be made available to you.
